What most HMS products get wrong
The gap is usually in three places. First, billing. Most products assume a simple encounter-to-invoice flow. Indian private hospitals typically have a more complex structure — procedures billed by department, pharmacy items billed separately, insurance authorisations running in parallel with cash billing, and TPA requirements that vary by insurer. A product that handles the simple case badly handles the complex case through workarounds.
Second, pharmacy integration. In most off-the-shelf HMS products, pharmacy is either a completely separate module or a bolt-on. Dispensing is not connected to the active patient file. Unbilled medications disappear into the gap between the ward and the billing desk. This is not a minor inconvenience — it is a revenue leak with a specific number attached to it.
Third, lab. Off-the-shelf products typically handle lab results as a manual entry step. Someone enters results into the HMS after the lab equipment produces them. The opportunity to connect the lab analyser directly to the patient record via HL7 or ASTM interface — eliminating the manual step and the transcription errors — is left on the table.
A hospital management system that does not connect pharmacy and lab to the patient record is not a hospital management system. It is a billing system with extra steps.
What a hospital in Delhi NCR actually needs

- OPD flow: Patient registration, token queue, doctor assignment, and billing — all in one flow without the front desk switching between systems.
- IPD integration: Admission to discharge connected to bed management, pharmacy, lab, and billing. The discharge summary triggers the final bill, not a separate process.
- Pharmacy dispense linked to patient file: Every item dispensed in the ward is attached to the patient record. Nothing can be dispensed without a corresponding patient entry.
- Lab results auto-push:Results appear in the treating doctor's patient view when they are ready — no paper walk, no delay.
- Live bed occupancy: Ward supervisors should not be calling each other to find out which beds are available. This should be visible in real time.
The build vs buy question
Off-the-shelf HMS products appear cheaper upfront. The licensing cost is visible; the customisation cost is not. Most Indian private hospitals that have gone through an off-the-shelf HMS implementation have a story about the customisation that cost as much as the product, the implementation partner that charged ₹50,000 per change request, and the workflow that still does not work the way the hospital operates.
